GPs, in Lombardy there are fewer and fewer professionals: it is the fault of politics
I am Andrea Pasini, an entrepreneur from Trezzano Sul Naviglio and I am increasingly convinced every day that the pandemic has unfortunately taught us very little. I’m talking about ours sanitary system. Not even these two years spent with Covid-19 have helped us to become aware of the flaws in our health system and in particular of the importance they had and have general practitioners.
The latter are the first link in the national health chain, as well as one of the most sensitive and important. A notion that extraordinarily still seems to escape our legislators. The number of general practitioners is decreasing and it has become difficult to find new recruits to replace doctors of retirement age.
Why, you may be wondering. The answer is very simple: the work is poorly paid and the bureaucratic burdens are endless. A maximalist general practitioner, that is, who reaches the maximum limit of 1,500 patients allowed, takes 38 euros gross per patient per year for a total of about 4,750 euros gross per month, from which 20% of withholding tax plus a other 20% tax. To this we must subtract the cost of the study: for example in Milan a minimum of 1500 euros per month is spent on rent, to which the bills, the cost of the secretary and possibly the nurse must be added.. Basically they earn less than 2000 euros a month.
Who would go to work like this, with this salary and all the associated risks? Who would undertake this profession knowing the amount of work that awaits him? Despite the negative proclamations of a certain political party and the bad press campaign that was launched, according to which general practitioners would be busy very little time, it must be said instead that this category works all day and very often is forced to take work home and work on weekends.
These professionals have to perform a mass of bureaucratic procedures that have now passed the clinical part, that is, the reports to the Ats, the reservations, the certificates, the portals to be filled in, all tasks that are impossible to perform during working hours. Then there are the patient’s phone calls, emails and Whatsapp messages to answer. A series of actions that for someone who does nothing all day seem easy to resolve but which instead for those who know work and sacrifice are a constant and tiring commitment. A doctor today goes on almost exclusively thanks to the passion he has for his work and the people he is committed to treating.. Today, more than ever, taking up this profession leads you to experience more criticality than positivity.
To an unfavorable media campaign are added the incorrect information in the bureaucratic and health fields that they have to interpret every day. I’m talking about green pass and vaccines, two fundamental tools on which however unacceptable chaos has undoubtedly been created. False expectations or misconceptions are created about the competences of general practitioners, which lead to friction with patients. Basically they are treated as if they were information points. In Milan and the province there are at least two cases of assaults a week: physical and vandalism at studies. Thus also the exposed ones have tripled in the last two years reaching the figure of 30 per day for 27,000 doctors in Milan and the province: these are requests for disciplinary sanctions against members by patients. Two years of pandemic have cracked the doctor-patient relationship instead of rediscovering the true value of these professionals.
Who would ever want to work in these conditions? The results are there for all to see, out of 600 scholarships in Lombardy, 530 participants presented themselves. A problem is that a law of the nineties that does not equate medical specialties to the training course for general practitioners, with repercussions in terms of the title (it is not a course managed by the university), and of the remuneration: if a trainee in fact, he receives 1400 euros net per month, those who attend the regional course get 800 euros gross. Now the question to be raised to the legislator is the following: does the local health system represent a value for politics? And if so, can you continue to manage it this way?
