How to fill out the friendly declaration form

The CID form consists of two pages, one for each driver involved in the accident. The information to be entered in the form is the following:

- Date and time of the accident
- Place of the accident (municipality, province, street, house number)
- Presence of injuries, even if minor
- Witnesses (if present, indicate surname, first name, address and telephone number)
- Data of the vehicles involved
- Make, model, license plate, chassis number, insurance number
- Driver (surname, name, date of birth, address, driving license number)
- Owner (surname, first name, date of birth, address)
- Dynamics of the accident
- Detailed explanations of how the accident occurred
- Indicate the direction of travel of the vehicles involved
- Indicate the position of the vehicles involved before and after the accident
- Indicate the presence of road signs
- Damage
- Describe the damage suffered by the vehicles involved
- Indicate the number of photos taken
Tips for filling out the CID form
- Fill out the form as soon as possible after the accident, while your memories are still fresh.
- Be as precise as possible when filling out the form, in particular in describing the dynamics of the accident and the damage suffered by the vehicles.
- If you disagree with the other driver's statements, clearly indicate your concerns on the form.
- Sign the form only after carefully reading all the information entered.
Finally, the completed form must be delivered within three days from the date of the accident, to your insurance company which will provide compensation for damages within 30 days of notification.

What happens if someone refuses to fill in the CID?
Following a possible road accident, it may happen that a motorist refuses to fill in the CID. This refusal obviously represents a sort of alarm bell; in fact, it may want to suggest the presence of an uninsured vehicle.
If the form is not completed, the insurance companies will have to carry out an appraisal to determine the dynamics of the accident and the damage suffered by the vehicles.
Be that as it may, in these situations all you have to do is fill out your own part of the form and try to get as much information as possible from the other party; It is necessary to take the vehicle's license plate and personal details. Photographs of the accident are also important in these cases. Once you have completed your document, you must deliver it to your insurance company.
