Doctors of the hospital named after N.I. Pirogov saved a girl with “double cancer”

Oncologists of the Russian Children's Clinical Hospital – a branch of the Russian National Research Medical University named after. N.I. Pirogov of the Russian Ministry of Health – helped a girl from Dagestan overcome “double cancer”. This is stated in a release received on Thursday, April 18, by Izvestia.

Doctors from the Center for Pediatric Oncology, Hematology and Transplantology of the Russian Children's Clinical Hospital of the Ministry of Health of the Russian Federation performed two operations on the little patient, since she had two unrelated tumors. Thanks to comprehensive treatment, the girl is now in remission for both diseases.

According to the hospital, in 2021 Zhannette from Dagestan began to experience pain in her leg, which intensified over time. Doctors suspected the girl had cancer and sent her to Moscow to the Russian Children's Clinical Hospital, a branch of the Russian National Research Medical University named after. N.I. Pirogov for further diagnosis and treatment.

“The child was admitted to our clinic in serious condition. The girl had difficulty walking due to severe pain in the lower back, and painkillers did not help her. She had an increase in body temperature, Jeannette was rapidly losing weight. In the lumbar region on the right, swelling and swelling were clearly visible,” said Ivan Skapenkov, head of the oncology department of surgical treatment methods at the Russian Children’s Clinical Hospital.

Due to the rapid deterioration of the patient’s condition, she urgently underwent a set of diagnostic tests: CT scan of the chest, abdominal cavity and pelvis, as well as MRI of the pelvis. The results of the examination confirmed the presence of a tumor of the wing of the right ilium and the right half of the sacrum.

In addition, in the girl’s right iliac vein, doctors discovered a tumor thrombus, the rupture of which could at any time lead to the death of the patient. Specialists from the Department of X-ray Surgical Methods of Diagnostics and Treatment urgently installed a special device – a venous filter. Installation of Jeannette’s device prevented the spread of the tumor (the blood clot also contained tumor cells) and allowed surgery to remove the tumor.

“We also performed a biopsy of the tumor, and a histological examination of the material allowed us to establish the final diagnosis: conventional osteosarcoma, osteoblastic variant. This is a rare and extremely aggressive malignant tumor arising from bone tissue. Its treatment requires complex multifactorial therapy: high-dose chemotherapy and surgical removal of the affected bone and adjacent structures,” Skapenkov continued.

The girl began receiving chemotherapy, and after the first course her condition improved significantly: the pain stopped bothering her, and the swelling in the right lumbar region decreased. And after three courses, control examinations confirmed a reduction in tumor size and the absence of metastases.

To perform the operation, the child was sent to the D. Rogachev National Medical Research Center for Pediatric Pediatric Orthopedics of the Russian Ministry of Health, whose specialists have the most experience in pelvic bone surgery in the country. The girl was operated on and the tumor and affected bone structures were removed.

After this, Jeannette returned to the Russian Children's Clinical Hospital, where she continued to receive antitumor treatment, at the same time underwent comprehensive rehabilitation, and did physical therapy exercises with doctors every day.

Thanks to the joint efforts of a multidisciplinary team, doctors were able to put the girl on her feet and teach her to walk again. And after completing courses of therapy, follow-up examinations and the onset of remission, the child was discharged under the supervision of doctors at the place of residence.

As the hospital clarified, six months later the girl began to complain of deterioration in her health, and the results of blood tests raised concerns among specialists. She was re-admitted to the Russian Children's Hospital for examination, during which doctors made a new terrible diagnosis – secondary myeloblastic leukemia.

“This is a different type of tumor, blood cancer, in which there is an uncontrolled production of immature bone marrow cells – blasts. Their accumulation reduces blood function and negatively affects the functioning of internal organs. For patients with this diagnosis, the only treatment method is bone marrow transplantation,” said hematologist, acting head of the department of hematology and chemotherapy No. 1 of the Russian Children’s Clinical Hospital, Mikhail Antoshin.

The girl was given chemotherapy courses to reduce the amount of abnormal cells in her blood, and then she was transferred to the bone marrow transplantation unit and began searching for a suitable donor. Doctors determined that Jeannette’s younger sister was 100% compatible, and a hematopoietic stem cell transplant was performed as soon as possible.

“The patient tolerated the transplant well, there were no complications. Now more than 160 days have passed since the operation, and complete engraftment of the graft is noted,” Antoshin continued.

Zhannette underwent comprehensive rehabilitation at the Federal Children's Center “Korablik” of the Russian Children's Clinical Hospital and continues to be monitored in the hematology and chemotherapy department. Specialists are monitoring the girl’s condition and adjusting immunosuppressive therapy. Doctors note no signs of relapse in either the primary tumor, sarcoma of the ischium, or the secondary myeloblastic leukemia; the girl is in remission.

In 2023, surgeons of the Russian Children's Clinical Hospital of the Russian National Research Medical University named after. N.I. Pirogov of the Russian Ministry of Health performed a unique operation to eliminate a rare congenital pathology – renal artery stenosis. The surgical plan drawn up by the doctors helped not only save the child’s life, but also preserve full kidney function.
